COMMUNITY HEALTH, LAST MILE
Seva Health Connect
Connecting Community to better health
WHY SEVA HEALTH CONNECT?
Public clinics, schemes, and trained workers are already in the district. What is missing is a trusted person who can sit on a courtyard mat, explain the next step, and stay until the appointment is kept.
Seva Health Connect trains that person. We do not replace the health system. We make it reachable—especially for women, adolescents, and households that screening never quite finds.
WHAT DOES IT DO ?
Adolescent Girls' Health
Educate villagers about hygiene, nutrition, sanitation, disease prevention, maternal health, and healthy lifestyles.
Basic Health Screening
Organize health camps and basic screenings, such as blood pressure and blood sugar checks, through qualified healthcare personnel.
Adolescent Girls' Health
Promote menstrual hygiene, nutrition, anaemia awareness, and access to appropriate health services for adolescent girls.
Healthcare Linkage and Referrals
Help people connect with government health centres, ASHA workers, ANMs, PHCs, CHCs, and hospitals for further care.
WHAT DOES IT DO ?
Four ways we close the last mile
Women
Antenatal support, anaemia, contraception counselling, and safe referral for complications.
Adolescents
Age-appropriate information, nutrition, and a confidential first conversation about health.
Prevention
Hygiene, immunisation reminders, NCD screening, and habits that keep households well.
Health linkages
ASHA, ANM, PHC, and specialist pathways made visible, practical, and followed through
A five-step approach to better health
01
Listen locally
We begin by understanding people, families and communities to identify the needs that matter most.
02
Train connectors
Community workers learn screening, counselling, and when to refer. Competence first; kits second
03
Meet people where they are
Household visits and courtyard sessions replace waiting rooms that never get reached.
04
Link into public care
Each person leaves with a named facility, a named worker, and the papers they need.
05
Follow until done
Registers close only when care is received — or a documented reason it was not.
Field notes, in photographs
Counts taken from household registers, training attendance, and closed referral slips for the 2025–26 season. Not modelled estimates.

“Someone came back the next week.”
Meera had been told, twice, to visit the PHC for anaemia. The first time she could not leave the shop. The second time she did not know which window to stand at.
A Seva connector sat with her, packed the old lab slip, and waited through the queue. Iron started that afternoon. A follow-up visit confirmed she was still taking it. That is the whole method: not a camp that leaves, a person who returns.
